Humana Launches Initiative to Address Social Determinants of Health

On March 4, Humana announced the launch of its value-based care program to address non-medical risk factors affecting its members. The Social Determinants of Health Value-Based Program will aim to “improve health outcomes by encouraging a more holistic approach to patient care that goes beyond traditional clinical treatment” and will complement existing value-based agreements. Social determinants are generally defined as the social and economic conditions impacting the overall health of groups and individuals. Humana’s program is designed to support clinicians in addressing issues such as food insecurity, social isolation, loneliness, and housing instability.  Humana will offer providers resources to address these challenges through increased reimbursement for enhanced care coordination based on patient screenings, documentation of findings, and connection to appropriate patient resources.
