On September 30, CMS published a final rule aimed towards improving the transfer of information between health care facilities by revising the discharge requirements that hospitals, critical access hospitals, and home health agencies must meet in order to participate in the Medicare and Medicaid programs. The rule creates discharge planning requirements to better inform patients and their families of their plan of care, and updates language relating to a patient’s right to request and receive medical records. This final rule becomes effective November 29, 2019.
