Texas Medical Disclosure Panel Approves New Informed Consent Forms for Certain Treatment

The Texas Medical Disclosure Panel has adopted new informed consent forms for medical treatment and surgical procedures, as well as for hysterectomies. These forms are used by physicians and other health care practitioners to disclose the risks and benefits of the proposed medical treatment or surgery and to obtain consent from the patient or the patient’s legally authorized representatives. Texas facilities must use the new forms beginning January 1, 2020.The notice of the adoption of the new informed consent forms along with the new forms were published in the July 21, 2019 Texas Register, on pages 3129 and 3130.
