Making Medical Information Card

Pages containing the front and back of such cards, are set out in PDF format. If you print out the front sheet, you can cut out the cards and write contact information on the back.

Or, if you print out both the front sheet and the back sheet, you can copy them back-to-back on the same sheet of paper and the card will have both a printed front and a printed back. You can copy them back to back by first printing the front from your printer, then turn over the printed front, place that sheet back in the printer, then print the back. Since the boxes have to be exact, make sure the paper is pushed firmly against the back of the printer for both sides. A commercial copy center can print these sheets back-to-back on card stock.

This material can be downloaded in either wallet-size or 3" by 5" format.

The cards can only be viewed with the Adobe  Acrobat Reader. If you don't already have it installed on your computer, you can download it for free from Adobe.
